What Is Voice Feminization?

The voice is produced when a vibrating source creates sound. The human voice is formed through the coordinated function of the lungs, the larynx (voice box) at the top of the trachea, and the oral and nasal cavities.

During respiration, air from the lungs passes through the trachea and causes the vocal folds in the larynx to vibrate, creating the basic sound (phonation). Vocal folds typically vibrate about 100–150 times per second in men, 200–250 times per second in women, and up to 400 times per second in babies.

This basic sound is then shaped in the throat, mouth, and nasal cavities to create an individual voice quality (resonance). Finally, speech sounds are produced by the coordinated movement of the lips, teeth, tongue, palate, pharynx, and nasal cavity, known as the articulatory organs (articulation).

What Is Voice Feminization?

Many people wish to change voice characteristics that do not match their gender identity, physical traits, or social expectations. This is especially common among transgender women, women with hormonal conditions or a history of hormone therapy, and women whose voices have deepened due to long-term smoking. These individuals may seek voice feminization to improve social comfort and psychological well-being.

In addition, professionals such as broadcasters, presenters, voice-over artists, politicians, executives, and academics may also request voice procedures to achieve clearer and more effective vocal communication.

With advances in medical science, vocal pitch can now be modified through surgical techniques. These procedures involve adjusting the vocal folds either through the mouth or via small incisions in the neck. To raise vocal pitch, the vocal folds may be shortened, thinned, or placed under greater tension.

Understanding Vocal Pitch and Loudness

Sound has three main characteristics: pitch, loudness, and timbre.

Pitch is what makes a voice sound high or low. It depends on the frequency of sound waves:

  • Higher frequency = higher (thinner) pitch
  • Lower frequency = lower (deeper) pitch

The pitch and loudness of the voice are influenced by three main properties of the vocal folds:

Length of the Vocal Folds:
Shorter vocal folds vibrate faster, resulting in a higher pitch.

Thickness of the Vocal Folds:
Thinner vocal folds vibrate more rapidly, producing a higher pitch.

Tension of the Vocal Folds:
Greater tension increases vibration speed, which raises pitch.

In healthy adults, the average fundamental frequency is typically:

  • Below 120 Hz in men
  • Above 200 Hz in women

Are the results of voice thinning operation permanent?

The results of voice thinning are usually permanent. However, the individual’s vocal habits, lifestyle and other factors can affect these results.

How long does the healing process take after voice thinning operation?

The healing process can vary from individual to individual, but usually takes several weeks. It may take time for the voice to fully recover and the desired tone to be achieved.

Is voice therapy necessary before voice feminization surgery?

Although voice therapy is not mandatory before voice feminization surgery, it is strongly recommended. Voice therapy not only focuses on increasing pitch, but also feminizes speech patterns, intonation, resonance, and articulation. Voice feminization surgery primarily raises the pitch of the voice but does not change a “masculine” speaking style on its own. When combined with therapy, voice feminization surgery produces much more natural and convincing results. Some patients achieve satisfactory outcomes with voice therapy alone and may not need surgery, while others consider surgery when therapy does not provide sufficient improvement.

Will my voice sound natural or artificial after voice feminization surgery?

When performed by an experienced surgeon, voice feminization surgery results in a natural-sounding voice, without a shrill or strained tone. The goal of the procedure is not only to raise the pitch, but also to preserve voice quality and fluency. With a good outcome, listeners will not realize that surgery was performed; they will simply hear a woman with a naturally higher-pitched voice. Continuing voice therapy after voice feminization surgery helps the voice sound more natural and feminine, as resonance and intonation are just as important as pitch. In the first few months after surgery, the voice may sound slightly hoarse or strained, which is normal and gradually improves over time.

How is voice feminization surgery performed? Will there be a scar on my neck?

There are several surgical techniques for voice feminization surgery, and the choice of method depends on the surgeon’s preference and the patient’s anatomy. The most commonly used technique is glottoplasty. In this method, the vocal cords are accessed endoscopically through the mouth, and the front portions are fused together using a laser to shorten the vibrating length of the vocal cords. With this technique, no incision is made on the neck.
